U.S. citizens and permanent resident are eligible. You must meet current credit and
other eligibility criteria. Applying with a credit worthy cosigner can give you a
better chance of approval

1st Hepatitis B Injection
TB Injection
CPR Certification
Stethoscope
BP Cuff
ASCP Phlebotomy Exam, 1st Attempt Only

PT Program -
Classroom and Clinical Training

Tuition does not cover:
All other immunizations
2nd and 3rd Hepatitis B Injection
State Application Fee
Medical Lab Assistant Exam
Background Check
Physical Exam, if necessary
Drug Screening, if necessary

1. 100% before the first class day.
2. 100% if class is cancelled by CAIOMS.
3. If 60% or less of the program is complete. Prorated Refund.
4. Nonrefundable if 60% or more of the program is complete.
